SSM Health at Home

SSM Health at Home is a nonprofit home health care agency providing service throughout southern Wisconsin. We are Medicare/Medicaid certified and accept a variety of insurance plans.

We offer a range of services – all with one goal – to keep our patients as safe and independent as possible at home. Through home health, hospice, home medical equipment, and a range of community services, we are able to support our patients throughout their health care journey.

Our mission is to promote optimal health and quality of life to those we serve through our exceptional, comprehensive care at home and in the community. Our primary goal is to keep people in their homes. To that end we offer many programs and services to help make that happen.
