RISE, formerly Center For Families

RISE emerged in April 2017 from a merger between Center for Families and Community Partnerships, Inc., two Dane County non-profits focused on community-based, strengths-driven services to children, families, and young adults. When Community Partnerships was established in 2000, the agency’s primary focus was providing mental health wraparound services to children and their families. Since 1981, Center for Families’ services and programs has focused on building strong, stable families and preventing child abuse. Together, the organizations have more than 50 years of experience providing services in our community, and serve more than 6,800 individuals (including children and families) annually. RISE believes in the power of human connection and the resilience that exists within everyone. Our services provide education, support, stabilization, and coordination of care to children, families, and individuals in our community.

Our Vision
By 2019, RISE will be well positioned as a leader in coordinated care for mental health and wellness services. Staff will embody the organizations’ values and reflect an integrated approach that puts the participants first. Staff and participants will share expectations for service delivery. The organization will have evolved from just being a good partner in Dane County to also framing equitable partnerships throughout the region. It will be well regarded for its stewardship of resources and its success in nurturing an inclusive and caring work environment that spans multiple physical locations.

We respect the unique needs and abilities of every individual and seek to leverage the strengths and assets that each person brings to the table. We also actively value caring for those who are providing services or serving in a support role to others in need.

RISE provides early childhood education, home visiting, and mental health services to children, adolescents, young adults and families.
Our programs include:
Case Management Services : Intensive case management services and wraparound support for children living outside of Dane County.
Children Come First Hospital Diversion Program : A short-term (30-90 days) program providing 24/7 crisis stabilization, in-home support, and diversion services to families who have a child at risk of an involuntary hospitalization.
Court-Ordered Evaluations : Serving as Dane County Human Services’ contract vendor for brokering various types of court ordered evaluations.
Early Childhood Initiative : A home visiting program offering comprehensive and community-based support to pregnant women and families with children aged 0-3.
Family Advocacy & Support : Parent and family support, including monthly Parent Meeting groups.
Mental Health Coordination Program : Partnership with Madison Metro School District, providing a single point of access to the MMSD Student Service staff to help them navigate the children's mental health system in Dane County.
Parent-Child Home Program : The Parent Child Home program prepares young children for school success by strengthening positive parent-child interaction that develops critical language and pre-literacy skills. Parents and children are in the program for two years, ages 2 - 4 years. Services take place in the family home.
Parenting for School Success : A Family Support Specialist works with families with children ages 3 - 5 to develop positive parenting skills, assist in creating goals and strategies, and assisting parents in understanding the growth and development of their children.
Respite Center : The Respite Center provides 24/7 child care and crisis intervention services for parents with children aged birth through 14 years. The program provides a nurturing and fun place for children while helping their parents cope during difficult times. The overall goal is to provide safety for children at the Respite Center and increase resiliency in parents and children to keep them safe when they go home.
Transition to Independence : Transitional case management services providing comprehensive support for young adults (ages 18-30) living with mental illness.
Welcome Baby : Welcome Baby visits new parents in their homes weekly to help them learn how to take care for their newborn, to teach them about child development and how to help their baby thrive through the first three years of its life.
